Anyone who wishes to participate as a volunteer in any capacity in the school, at any school event or field trip must first have a Diocesan Background Criminal Investigation Check. This clearance is valid for three (3) years from issue date. At this time there is no cost for this to be completed. The completed disclaimer form must be submitted through your child's school or the Regional Office only, not through another agency. Please note the form must be notarized; notary services are available at either school or the Regional Office. A legible copy of a government issued photo ID with a date of birth must accompany the disclaimer. The BCI disclaimer form is available at either school or can be downloaded from the link below.
